Hepatitis B Vaccination Series Completed with Verification (1st, 2nd, 3rd) - Get started on this first because it takes about 6 months to complete |

Complete application and documentation for admission to MSU-GF College of Technology-this is separate from applying to the dental hygiene program |

Dental Hygiene Application Packet-can download off of website or have Holly Schott email or send you one. Make sure it’s current as it changes periodically |

| Complete Application Evaluation with the grades you received. (A=4, B=3, C=2) | |
Verification of Occupational Experience in an Oral Health Field |

Official Transcripts for all completed secondary education (Send transcripts to Registrar- No need to include in packet) |

If you have previous degrees/certificates please photocopy diplomas and include in the packet |
