Some students with disabilities are eligible to receive their text books in an alternate format. If you have met with Disability Services and been determined eligible for this accommodation, and if you would like to receive your text books in electronic format, please complete the information below (all fields are required in order to process your request) and submit to Disability Services. You will need to submit a separate form for each text book you are requesting. Publishers generally require at least three weeks for processing these requests (often more near the beginning of the semester), so it is important to submit these requests in a timely fashion.
