All of these forms must be completed and returned to the Financial Aid Office. You can bring them to the office; mail them to: MSU-Great Falls COT, Financial Aid Office, 2100 16th Ave South, Great Falls, MT 59405; or fax to: (406) 771-4317 or (406) 771-4329 attn. Financial Aid. Please do not complete these forms unless you have received notification from the Financial Aid Office to do so. Financial aid eligibility will be determined after the requested forms are returned to the Financial Aid Office. If you have not completed your FAFSA application please do so at www.fafsa.ed.gov . Academic Year 08-09 [Fall 2008, Spring 2009 and Summer 2009]2008-2009 Student Data Form [PDF]: Required for all students. The Student Data Form is used by the Financial Aid Office to gather additional information about your plans for the school year. This form is required for all students wishing to receive financial aid at MSU-Great Falls College of Technology. The Student Data Form will ask you to estimate how many credits you will be taking for the entire school year. This form will also ask you to list any resources, other than Federal financial aid, that you will or you expect to receive during your term(s) of enrollment. You will also need to list any other institutions that you have attended or will attend between July 1, 2008 and June 30, 2009.
2008-2009 Income Verification Form [PDF]: Required for all students. The Verification Worksheet is used by the Financial Aid Office to verify the information that you have submitted on the FAFSA application. This form is required for all students wishing to receive financial aid at MSU-Great Falls College of Technology. On the Verification Worksheet list the people that you (and your parents if you are dependent) will support between July 1, 2008 and June 30, 2009. Provide a summary of your household's 2006 income on the form. The second page of the Verification Worksheet requires you to list untaxed income you in 2007. The appropriate signatures are required to process the form.

One of the most common items on this form will be BAS/BAQ, which is listed on military member's LES. Another common form of untaxed income is child support received in 2007. The Low Income Statement is used by the Financial Aid Office to determine how you met your expenses in 2007. This form is requested when the income t reported on your FAFSA or Verification Form appears to be unusually low. If you are requested to fill out this form you will need to complete both columns, if you are married your spouse's information is also required. If you are considered a dependent student by the Financial Aid Office your parents will need to complete the second page of the form.

Miscellaneous FormsWork Study Waiting List [PDF]: If you are interested in work study and it was not on your award letter, complete this form out. This form must be submitted to the Financial Aid Office to etermine if you are eligible for work study and if there are funds available to award to you.
Satisfactory Academic Progress Appeal Form [PDF]: If you are put on Financial Aid Suspension and you think that you meet the qualifications to do an appeal to the Financial Aid Office, complete this form. Please be sure if you complete this form you also include a personal statement and any relevant documentation. Credit Change Form 2007-2008 [PDF]: The credit change form is the form that you will need to complete if you decide to take a different number of credits than you have originally told the Financial Aid Office. Summer 2008 hours will not be adjusted until you have registered for Summer classes. This form needs to be returned to the Financial Aid Office as soon as possible.
Additional Loan Request Form [PDF]: This form is used to ask for additional loan funds from the Financial Aid Office . Examples of when you would complete this form would be if you have earned over 30 credits in the middle of the school year, or you have previously rejected your student loan and now you would like to request it. Please DO NOT fill out this form with out first checking with the Financial Aid Office to determine if you are eligible for additional money. If you are eligible for additional funds you must complete the form in its entirety.. This form needs to be returned to the Financial Aid Office.
