Application for Admissions [PDF]

Mail, fax, or return your application, along with a $30.00 non-refundable application fee* to: Admissions, MSU-Great Falls College of Technology, 2100 16th Ave S, Great Falls MT  59405. Be sure to complete all other  Admissions Requirements.

*If you have previously attended any branch of MUS School, this fee may be waived.

Returning (Readmit) [PDF]

If you have previously attended MSU-Great Falls College of Technology, but have not attended for one (1) consecutive semester, you are considered a Returning or Readmit Student and must complete a Readmit Application.  If you have not attended for more than five (5) years, you must complete the full Application for Admissions above.

Transcript Request from Previous School [PDF]

If you have applied for admissions and need a previous school (high school, college, or university) to send transcripts to the College of Technology, print this form out and mail to the school you need.

Utilize these forms once classes have started (or if you are unable to change your own schedule on Banner Web).  To add a class without an override authorization form, the class must be open (have available enrollments) and the form must be signed by the instructor.  Submit to Admissions.  Last dates to Add and Drop courses are published on the Academic Calendar.

Override Authorization Form [PDF]

Utilize this form to gain registration in a class that is closed, conflicts with another class, or has restricted entry.  The form MUST be signed by the appropriate instructor, and in some cases will also require the Department Chair signature.  Submit to Admissions.

Admissions Academic Progress Appeal [PDF]

Submit completed form, along with all required documentation and a readmit application, to the Registrar in order to be considered for readmission to the college after completing a mandated period of academic suspension.

Petition to Register for Excess Credits [PDF]

Complete this form to request registration for credits above the maximum. This petition must be signed by both the student's academic advisor and Assistant Dean of Student Central and turned in to the Admissions office prior to being allowed to register for additional credits. This petition allows the student to register for no more than the approved number of credits. A student who is in good academic standing may enroll for a maximum of 19 credits per semester without special approval. A student who is on academic probation may register for a maximum of 13 credits per semester without special approval.

Complete this form to be considered for an exception to published withdrawal and refund policies. Submit completed form, along with all required documentation, to the Registrar.

Change of Program Form [PDF]

This form must be signed by student & new faculty advisor and returned to the Admissions/Records Office.

Change of Address/Telephone Number Form [PDF]

It is important to maintain accurate address/telephone information while attending the College. Because of our integrated student records data base, other departments use these addresses to mail bills, refund checks, and other important documents or notices.  Change of Address/Telephone Number forms are also available at the Information Desk and in admissions and can be submitted to either.

In the event that your legal name has changed complete and submit this form to the Admissions & Records Office along with a photo copy of your new Social Security card.

Incomplete Grade Request Form [PDF]

An Incomplete "I" grade is given at the discretion of the faculty when course work has been satisfactory, but for unavoidable mitigating circumstances, the student cannot complete the work by the end of the term.  The student should consult with the instructor and if an incomplete is warranted, complete a Request for Incomplete Grade form stating the circumstances preventing completion and conditions under which the work will be completed.  This form must be submitted by the instructor with the final grade roster or the student will receive an "NR" grade until the paperwork is complete.

Transcript MUS Core [PDF]

Use this form to get the Montana University System Core transcripted without applying for Graduation.  The $5 fee must be submitted with this form to the Cashiers & be accompanied by the Program Advisory Sheet and your Academic Advisor’s signature.

A written request, with signature, must be received by Admissions & Records for processing of an Official Transcript. Acceptable requests are hand-written mailed or faxed requests and requests submitted on the College's Official Transcript Request Form.

Transmittal Application Form [PDF]

In order to assist undergraduate students who transfer between units of the Montana University System; or enroll in coursework at more than one unit of the Montana University System in the same semester the Montana Board of Regents authorizes a “single admissions file” that will follow the student throughout the System, much like a patient's medical records, regardless of which campus(es) the student enrolls in. The file will be supplemented with additional information, when that information is part of the admissions and transfer decision process at the student's new institution. There is an $8 fee for this service to cover the cost of copies and postage.

If a student decides to attend another unit of the Montana University System, that student must complete this Request for Transmittal of Application Materials and submit it to the unit of the Montana University System that the student most recently attended, hereinafter referred to as the sending institution. The sending institution will prepare a certified copy of the student's admissions file, and pass it along to the unit or units identified in the admissions file transmittal form. The certified copy will include:

1. a copy of all the documents that make up the initial admissions file, described under Procedures, paragraph A, including transcripts from other educational institutions;

2. any additional admissions information added by subsequent units of the Montana University System, if the student has attended more than one unit;

3. a certification statement from the sending institution;

4. an official transcript of the coursework completed by the student at the sending institution;

5. for transfer students, an analysis of the general education coursework completed by the student at the sending institution. If the student has completed the lower division coursework in the approved general education program at the sending institution, which satisfies the transfer expectations in Board Policy 301.10 , under paragraph II.A, Procedures, a statement to that effect will be included in the certified information. That statement will become a permanent part of the admissions file passed on to other institutions. If the student has not completed such a program, that fact should also be part of the certified information.

6. any additional and relevant information that was part of the admissions decision process at the sending institution or any other unit of the Montana University System that the student might have attended.

A student may be required to supplement the information in the initial admissions file, once it is submitted to the new institution, in order for the new institution to determine the admissions status of the student. That is especially true if the initial admissions file was assembled at a two-year institution and the student is now seeking admission to a four-year institution. Examples of such supplemental information may include, but are not limited to, the following:

1.        ACT or SAT scores and a high school transcript;

2.        immunization records;

3.        residency information.

This policy is only intended to assist students in the collection and transmittal of information that is part of the documentation necessary for admission to the units of the Montana University System. It does not guarantee admission to the new institution or institutions that receive the admissions file information. That decision is still left up to the individual units of the Montana University System.

Because of storage limitations, any admissions file assembled under this policy only has to be retained by each campus that comes into its possession for two (2) years from the date of its receipt.

Update Application Form [PDF]

This form is to be used if you applied for admission for the previous semester and did not register for classes or for the next semester and have decided to attend the current semester. For example if you applied for spring 2007 semester and did not register and want to register for summer 2007 classes or if you applied for fall 2007 and would like to register in summer 2007.

MSU—Great  Falls  College  of  Technology  (MSUGF)  recognizes  that  learning  occurs outside  of  the  college  setting.  The  outcome  of  this  learning  is  often  the  acquisition of  skills  and/or  knowledge  which  may  be  equivalent  to  learning  at  MSUGF  and other  institutions  of  Higher  Education.  MSUGF  may  award  credit  for  this  learning through  the  MSUGF  Experiential  Learning  Policy.
